Southern Crescent Family Practice is dedicated to providing you with the highest quality medical care. We accept many Commercial Insurances, select Medicare and Medicaid Plans, all major credit cards.  We also offer other payment installments and options.

Additional Fees: 

There is an additional fee for completion of forms for use outside of the patient's medical record, including:

  • Parking Affidavit​

  • Life Insurance

  • Family Medical Leave Fee: $150.00

  • Missed Appointment Fee: Starts at $75 up to $450 depending on appointment type
